Patient Registration

To speed up the registration process at your first appointment, please print and fill out one of the following registration forms (the first form is for adult patients, the second for pediatric patients) as well as the New Patient Questionnaire, Patient Medication List, and Medical Records Release forms and bring the completed forms with you to our office. Patients new to our clinic should expect their first appointment to last between one and three hours depending on the level of skin testing necessary.

Please do not take any medication that contains an antihistamine for 5 days prior to your first appointment. This does not include inhalers, nasal sprays or prednisone. It does include prescription and over-the-counter oral antihistamines. If you take an antihistamine within 5 days of your appointment please contact our office because we may be required to reschedule your appointment. Unfortunately skin testing is not as effective when an antihistamine has been taken.
